The original checklist is filled with your answers. The PDF stays two pages. Please keep answers concise; the form will ask you to shorten any answer that cannot fit legibly. Your typed name is an acknowledgment, not a digital certificate signature. KOSTER staff fields remain blank. Attach the downloaded PDF to your email manually.
